Applying for site membership and UKARA License

At last I'm not working a weekend so I will be at my local site* this weekend, and I would like to apply for site membership and my UKARA license. Would someone please be kind enough to let me know what I 'typically' will need - I assume a passport style photograph(s), for of Id, and of course cash :+)

General rule is 2 form of ID. One with a photo and a second separate one with your name and address. Plus a photo of yourself for them to use for your card and you're done.
