UKARA form question

Hi guys,

probably me just being stupid but I don't want to send the form off with the wrong information on it.

At the bottom of the page it asks for your ID .. do you just put in your Passport number, Drivers License, Birth certificate etc that it asks for?
Also the address section beneath it ..

That's for the I'd you show the the skirmish site that stamps you're form.
The skirmish site stamp your form on your 3rd visit. Then you send the form to a ukara registered retailer.
